New Patient Registration

We welcome new patients. If you live within our practice area you may register with the surgery. We register patients every day that the surgery is open. If you are coming to the surgery you will find it less busy in the afternoons.

All our new patients will be asked to complete a simple medical questionnaire, so that the doctors have some information about you immediately.

You will also be asked to fill out a Family Doctor Service Registration Form for each person registering with the surgery. If you have a medical card please bring it with you as this helps to obtain your previous records.

To complete your registration it is necessary for each person to supply a photograph ID and two current utility bills or correspondence showing your current address. If you are unable to provide this information your registration may be declined. Please contact the surgery if you have any questions.
